Download & print the form
It is a one-page official written agreement / ballot. You can type Section 2 into the PDF before printing, or print it blank and fill it in by hand. No printer? Call or text us and we will bring you a copy.

Mark RECALL & sign in blue ink
- Check RECALL next to Ralph Viera, Jose Sanchez, and Chris Koval.
- Fill in your name, Sunset Lakes address, and neighborhood (for example, Grand Key).
- Sign by hand in BLUE INK and write the date. The form requires a handwritten blue-ink signature.
- One form per home: exactly one designated voting owner per parcel signs.
